Regional Health Connector

Are you passionate about making a positive impact on the health and wellness of our region?  Chaffee County BOCC, Public Health department is accepting applications for a full-time, grant-funded Regional Health Connector program coordinator.  The Regional Health Connector will support existing partnerships by strengthening connections between primary care and community organizations, coordinating activities to reduce fragmentation in the health system, and integrating clinical and community-based strategies to address local priorities, social determinants of health and health equity.  This is a regional position, and work will take place in Chaffee, Lake, Fremont, and Custer Counties.  Minimum qualifications include a Bachelor's degree or 4 years of experience in a health-related field, computer skills in Microsoft Outlook, Word, Excel and PowerPoint, good verbal and written communications skills, valid CO driver's license.  This is a temporary grant-funded position; current term is thru January 2019.  Exempt; full-time, full benefits; requires some evenings and weekends.  Click here for the application Application;  click here for job description Job Description.  Applications and resumes may be emailed to bvenes@chaffeecounty.org  Open until filled.  EOE


Maintenance Worker

Self-Sufficiency Specialist

Family Resource Coordinator

Assistant Deputy Treasurer
